Which M79.60 code do I assign?
Subcategory M79.60 splits on two facts at once: which limb the note names and which side of the body it names. M79.605 is the point in that run where the record has supplied both, the leg and the left.

Why specificity matters beyond the chart: risk adjustment, DRG assignment and denial exposure all follow from which code is selected.

FAQ
Yes. M79.605 is a valid billable code and requires no further characters. It is the most specific code available when the record names the leg and the left side but names no segment of the leg. The FY2027 tabular carries Pain in left lower limb NOS as its inclusion term.
The difference is one word in the record. M79.605 is pain in the left leg. M79.606 is pain in the leg with no side stated, under the inclusion term Pain in lower limb NOS. Section I.B.13 of the FY2027 Official Guidelines assigns the code for the unspecified side only where the side is not identified in the medical record. Where the note says left, section I.B.2 requires the code that carries the detail. Subcategory M79.60 runs both limbs through one series of sixth characters: 1, 2 and 3 are the arm, 4, 5 and 6 are the leg, and 9 is the unspecified limb. The digits are not a specificity ladder. M79.606 is not a parent of M79.605.
Step down from M79.605 as soon as the note names a segment of the left leg. Left thigh pain is M79.652 in subcategory M79.65. Left calf, left shin and left lower leg pain are M79.662 in subcategory M79.66. Both codes are more specific than M79.605 and displace it under section I.B.2. The laterality digits differ from the M79.60 run: 1 is the right side, 2 is the left side and 9 is unspecified at both M79.65 and M79.66. The knee is not in M79.6 at all. Left knee pain is M25.562.
Left leg pain is coded to sciatica where the provider documents sciatica. Radicular pain in the sciatic distribution on the left is classified to M54.32. Low back pain documented together with the left sided sciatica is classified to M54.42 instead, under the Excludes1 note at M54.3 for sciatica with lumbago. Sciatica attributed to an intervertebral disc disorder is classified to the M51.1- codes. A documented lesion of the sciatic nerve on the left is classified to G57.02. Both of those routes follow Excludes1 notes at M54.3. M79.605 is correct only where no such diagnosis has been established.
Two vascular diagnoses and a compartment syndrome also displace M79.605. Exertional left calf pain relieved by rest and attributed to peripheral arterial disease is classified to I70.212, atherosclerosis of native arteries of extremities with intermittent claudication, left leg. The same disease with pain at rest is classified to I70.222. Acute left leg pain and swelling attributed to deep vein thrombosis is classified in the I82.4- range. Within it, I82.402 covers the unspecified deep vein of the left lower extremity, I82.4Y2 the proximal segment and I82.4Z2 the distal segment. Nontraumatic compartment syndrome of the left lower extremity is M79.A22. Traumatic compartment syndrome of the left lower extremity takes a T79.A22- code with a seventh character for the encounter. Chapter 13 lists traumatic compartment syndrome, the T79.A codes, as a chapter level Excludes2.
M79.605 reaches neither. M79.605 carries no CMS-HCC v28 mapping. No other code in subcategory M79.6 carries one either. Naming the left side does not move a Medicare Advantage risk score. Risk adjustment and inpatient severity are separate questions. As a principal diagnosis M79.605 groups to MS-DRG 555 or 556 in MDC 08, a pair that splits on MCC alone with no CC tier between them. A left leg workup can reach three v28 categories. They are HCC 267 for deep vein thrombosis and pulmonary embolism, HCC 263 for atherosclerosis of arteries of the extremities with ulceration or gangrene, and HCC 264 for vascular disease with complications. Each is reached through the vascular diagnosis code.
Sequencing turns on why the patient is there. Where the encounter is for pain control or pain management, the category G89 code is sequenced first and M79.605 follows as the site of pain. Where the encounter is for any other reason and no related definitive diagnosis has been established, M79.605 is sequenced first and the G89 code follows. A G89 code is not assigned at all where the underlying definitive diagnosis is known, unless the reason for the encounter is pain control or pain management. Section I.B.4 of the FY2027 Official Guidelines accepts a symptom code for reporting when a related definitive diagnosis has not been established. It notes that Chapter 18 contains many, but not all, codes for symptoms. The setting changes only how uncertainty is handled. An outpatient encounter codes the confirmed diagnosis alone, under section IV.H. An inpatient discharge diagnosis documented as probable or suspected may be coded as though confirmed.
M79.605 is unchanged in the FY2027 update, effective 1 October 2026. The FY2026 update, effective 1 October 2025, also left the descriptor, the inclusion term Pain in left lower limb NOS and the Excludes notes at M79.605, M79.60 and M79.6 unchanged.
