Which M25.55 code do I assign?
Subcategory M25.55 splits on one axis alone: the side of the body. The prior question is whether a cause for the pain has been documented, because a documented cause moves the encounter out of M25.55 entirely.

Why specificity matters beyond the chart: risk adjustment, DRG assignment and denial exposure all follow from which code is selected.

FAQ
The code for left hip pain is M25.552, pain in left hip. It is a valid billable code and requires no further characters. The right hip is M25.551 and a record that never names a side supports only M25.559, pain in unspecified hip. M25.552 applies when the pain is placed in the hip joint and no diagnosis explaining it has been established by the end of the encounter.
A diagnosis replaces M25.552 as soon as the provider documents a cause. Osteoarthritis of the left hip is M16.12 when primary, M16.32 when it results from hip dysplasia and M16.52 when post-traumatic. An osteophyte reported on imaging without a stated osteoarthritis diagnosis is M25.752. Trochanteric bursitis is M70.62, which carries trochanteric tendinitis as an inclusion term. Gluteal tendinitis is M76.02. A labral lesion is classified to M24.152 when no injury is documented and to the S73.19- sprain codes when an injury is. Section I.B.4 of the Official Guidelines accepts symptom codes when a related definitive diagnosis has not been established. Section I.B.5 states that signs and symptoms associated routinely with a disease process should not be assigned as additional codes.
No. The Alphabetic Index treats the two words differently. Under the main term Pain, the subterm groin reads see Pain, abdominal, lower. That path leads to R10.30 for the unspecified site, R10.31 for the right lower quadrant and R10.32 for the left. A note reading left groin pain and nothing else supports R10.32, not M25.552. Where the provider documents hip joint pain and uses groin only to locate what the patient feels, M25.552 is the code. A query resolves the ambiguity where the note supports both readings.
No. M25.552 carries no CMS-HCC v28 mapping and no code in subcategory M25.55 does either. Documenting the side does not change a Medicare Advantage risk score. Several entries in the hip differential do not change it either: hip osteoarthritis at M16.12, trochanteric bursitis at M70.62 and a non-traumatic labral lesion at M24.152 each carry no v28 HCC. Idiopathic aseptic necrosis of the left femur at M87.052 is the exception. It maps to HCC 92, bone, joint, muscle and severe soft tissue infections and necrosis, where it mapped to HCC 39 under v24.
The note must show three things at once. The pain is located in the hip joint rather than in the groin, the thigh, the buttock or the low back. The record names the left side. No diagnosis explaining the pain has been established by the end of the encounter. Any one of the three failing moves the encounter to a different code.
M25.559 is treated as a documentation failure because the examining provider has already identified which hip is involved. Section I.B.13 of the FY2027 Official Guidelines states that codes for the unspecified side should rarely be used. It permits them only where the record cannot establish the affected side and clarification cannot be obtained. The same section permits the side to be taken from the documentation of clinicians other than the patient's provider. Therapy notes and imaging reports are therefore in scope before a query is raised. M25.559 is also not a bilateral code. Pain in both hips is reported as M25.551 and M25.552 together, because subcategory M25.55 carries no bilateral code.
As a principal diagnosis M25.552 groups to MS-DRG 555 or 556 in MDC 08 under the FY2027 MS-DRG v44 grouper, signs and symptoms of musculoskeletal system and connective tissue with or without MCC. The pair splits on MCC alone and there is no CC tier. Where the admission establishes a cause, that cause is sequenced as the principal diagnosis and the case leaves the pair. Hip osteoarthritis groups to MS-DRG 553 or 554 and a hip sprain at the initial encounter groups to MS-DRG 537 or 538. The selection rule is the same in the outpatient setting. The difference lies in how uncertainty is handled. An outpatient encounter codes only what is confirmed. An inpatient discharge diagnosis documented as probable or suspected may be coded as though confirmed under UHDDS rules.
No. The FY2026 update, effective 1 October 2025, left the descriptor of M25.552 and the Excludes notes above it unchanged. The FY2027 update, effective 1 October 2026, adds, deletes or revises no code in category M25. It changes no note in category M25.
