Which M54.5 code do I assign?
The M54.5 family splits on what the note names as the character of the pain rather than on how severe the pain is. Reserve M54.50 for a documented complaint of low back pain that carries no radicular pattern, no vertebrogenic attribution and no established structural cause.

Why specificity matters beyond the chart: risk adjustment, DRG assignment and denial exposure all follow from which code is selected.

FAQ
Yes. M54.50 is a valid billable code for HIPAA-covered transactions and for Medicare claims.
Use the M54.4- codes when the note documents sciatica alongside the low back pain. Subcategory M54.5 carries an Excludes1 note for lumbago with sciatica. The two are not reported together for the same condition.
No. M54.50 maps to no HCC category under CMS-HCC v28, and it mapped to none under v24. It makes no contribution to a Medicare Advantage risk score.
Yes. M54.50 covers acute and chronic low back pain when the documentation does not specify a structural etiology. When the provider documents the low back pain as chronic, also assign G89.29, Other chronic pain, under guideline Section I.C.6.b.1.(b). M54.50 does not say whether the pain is acute or chronic. Sequence G89.29 first when the encounter is for pain control or pain management. Otherwise list M54.50 first and G89.29 after it.
That depends on what the provider has documented. If the provider has correlated the imaging finding to the patient's symptoms and recorded a disc-related diagnosis, code the structural condition. If the finding is incidental and the clinical diagnosis remains nonspecific low back pain, M54.50 is appropriate.
In the outpatient setting, code to the highest degree of certainty. If the workup has identified a structural cause, code the identified condition rather than the symptom. On the inpatient side, UHDDS principal diagnosis rules apply. If an underlying condition is confirmed during the stay, that condition becomes the principal diagnosis in place of M54.50.
M54.50 is not reported alongside a structural spinal diagnosis when the low back pain is routinely associated with that diagnosis. Guideline I.B.5 states that signs and symptoms associated routinely with a disease process should not be assigned as additional codes. Subcategory M54.5 also carries Excludes1 notes for lumbago due to disc displacement (M51.2-) and for M51.360 and M51.370.
M54.50 is unchanged in the FY2027 update. The FY2027 codes and tabular addenda make no change at M54 or M54.5. The CDC NCHS FY2026 addenda also published no Excludes note changes, no new child codes and no tabular instruction revisions for M54.50 or for the M54.5 subcategory.
