M54.16

M54.16: Radiculopathy, lumbar region

The ICD-10-CM code for lumbar radiculopathy when the provider documents irritation or compression of a lumbar nerve root producing radicular pain, numbness or weakness. No intervertebral disc disorder or spondylosis is documented as the cause.

Code Family

M54.1: Radiculopathy, classified by spinal region.

HCC Mapping (v28)

No HCC mapping: M54.16 does not map to any CMS-HCC v28 category, and carried no mapping under v24 either.

Condition

Lumbar radiculopathy

Family Specificity

Billable region-specific child code. The parent M54.1 is non-billable. Only the region-specific children are reportable.

Effective Date

Valid in the FY2026 code set and unchanged in the FY2027 update.

Which radiculopathy code do I assign?

The M54.1 subcategory assigns radiculopathy by spinal region. The documented level of the nerve root drives the code. Before choosing any of these, confirm that no intervertebral disc disorder or spondylosis is documented as the cause, because those findings move the encounter to the M51.1-, M50.1- or M47.2- codes instead.

When documentation says…
Code
Radicular leg pain and numbness from a lumbar nerve root, no disc disorder identified
M54.16
Use when the provider documents radiculopathy localized to the lumbar region without specifying lumbosacral involvement. The note must state radiculopathy as the diagnosis and show lumbar nerve root findings such as dermatomal pain, sensory change, weakness or a positive straight-leg raise.
Radiculopathy documented as lumbosacral
M54.17
Use when the documentation specifies the lumbosacral region rather than the lumbar region alone. If the site is ambiguous between the two, query the provider before assigning either code.
Radiculopathy of the sacral or sacrococcygeal region
M54.18
Use when the documented nerve root involvement is sacral or sacrococcygeal.
Radiculopathy at the thoracolumbar junction
M54.15
Use when the provider documents the thoracolumbar region as the site.
Radiculopathy of the thoracic spine
M54.14
Use when thoracic nerve root involvement is documented.
Radiculopathy at the cervicothoracic junction
M54.13
Use when the documented site is cervicothoracic.
Radicular arm pain from a cervical nerve root, no disc disorder identified
M54.12
Use when cervical radiculopathy is documented. If a cervical disc disorder is the documented cause, an M50.1- code applies instead.
Radiculopathy of the occipito-atlanto-axial region
M54.11
Use when the documentation places the nerve root involvement at the occipito-atlanto-axial level.
Radiculopathy documented with no spinal region stated
M54.10
The residual code, used only when the record gives no region. Do not default to it when lumbar involvement is documented, because that loses the regional specificity the family is built to capture.

Tabular notes

EXCLUDES1 AT THE M54.1 SUBCATEGORY LEVEL

Neuralgia and neuritis NOS (M79.2). Radiculopathy with cervical disc disorder (M50.1). Radiculopathy with lumbar and other intervertebral disc disorder (the M51.1 codes). Radiculopathy with spondylosis (the M47.2 codes). These are Excludes1 notes. When a disc disorder or spondylosis is the documented cause of the radiculopathy, report that code instead of M54.16. M54.16 is not reported with an M50.1-, M51.1- or M47.2- code for the same radiculopathy. Guideline I.A.12.a allows both codes when the conditions are unrelated.

EXCLUDES1 AT THE M54 CATEGORY LEVEL

Psychogenic dorsalgia (F45.41). This parent-category exclude propagates down to M54.16. When back symptoms are documented as psychogenic in origin, report F45.41 rather than M54.16.

NO EXCLUDES2, USE ADDITIONAL OR CODE FIRST

The FY2027 tabular carries no Excludes2 notes, Use Additional instructions or Code First requirements at the M54.16 code level. Chapter 13 carries a general note to use an external cause code following the code for the musculoskeletal condition, if applicable, to identify the cause.

FY2027: EFFECTIVE 1 OCTOBER 2026

None. The FY2027 codes addenda add, delete or revise no code in subcategory M54.1. The FY2027 tabular addenda change no note at M54 or M54.1. M54.16 is unchanged in the FY2027 update.

Operational & financial impact

Why specificity matters beyond the chart: risk adjustment, DRG assignment and denial exposure all follow from which code is selected.

CMS-HCC v28 (PY2024+)

No HCC mapping applies. M54.16 does not map to any Hierarchical Condition Category in CMS-HCC Model Version 28, which is fully operative at 100% weight for Medicare Advantage payment year 2026.

Symptom-level musculoskeletal codes, including regional pain and nerve complaint codes, sit outside the CMS-HCC risk adjustment model in both v28 and the prior v24 model. Reporting M54.16 does not contribute to a beneficiary's risk score or generate HCC credit for a Medicare Advantage plan.

MEDICARE AND PAYER COVERAGE

There is no National Coverage Determination specific to M54.16. The code supports medical necessity documentation for lumbar imaging, electrodiagnostic studies such as EMG and nerve conduction testing, physical therapy, epidural steroid injections and pain management referrals.

WHERE SPECIFICITY IS LOST

Two selections put a lumbar radiculopathy claim at odds with the tabular. The first is defaulting to M54.10 when the lumbar region is documented. The second is reporting M54.16 alongside M51.1- when a disc disorder is the documented cause.

FAQ

When is M54.16 the right code instead of M51.16?

M54.16 applies when radiculopathy is documented without disc pathology, or when the cause has not been identified as a disc disorder. Once a lumbar or other intervertebral disc disorder is documented as the cause of the radiculopathy, M51.1- applies and the Excludes1 note at M54.1 prohibits reporting M54.16 for the same radiculopathy.

Can M54.16 and M51.16 be reported together?

M54.16 and M51.16 are not reported together for the same radiculopathy. The Excludes1 note at M54.1 bars M54.16 when a disc disorder is the documented cause. Subcategory M51.1 carries the reciprocal Excludes1 note for lumbar radiculitis NOS (M54.16). That chart is coded to M51.16 alone.

How is M54.16 different from low back pain and from lumbago with sciatica?

M54.16 applies when the documentation specifies radiculopathy as the nature of the complaint. Low back pain without documented radiculopathy is coded to M54.50, M54.51 or M54.59 as appropriate. Sciatica documented with lumbago is coded to the M54.4- codes.

Does M54.16 carry risk adjustment weight?

No. M54.16 carries no HCC designation in CMS-HCC v28 or v24. Reporting it does not raise a beneficiary's risk score or generate HCC credit for a Medicare Advantage plan.

What happens if a disc disorder is identified during an inpatient stay?

If further evaluation during the admission identifies a disc disorder as the cause of the radiculopathy, code the disc disorder at discharge under the UHDDS definitive diagnosis rules. If the cause remains undetermined at discharge, M54.16 remains appropriate.

Should M54.16 be reported alongside a definitive diagnosis?

M54.16 is not reported alongside a definitive diagnosis when the radiculopathy is routinely associated with that diagnosis. Guideline I.B.5 states that signs and symptoms associated routinely with a disease process should not be assigned as additional codes, unless otherwise instructed by the classification.
