Which urinary incontinence code do I assign?
ICD-10-CM classifies urinary incontinence by type rather than by severity, and the type codes sit in more than one chapter. Assign R32 only when the record does not identify the incontinence as stress, urge, mixed, functional or nonorganic enuresis.

FAQ
Yes. R32 is a valid billable code for HIPAA covered transactions. It is a complete three character code and takes no further character.
R32 applies when the type of urinary incontinence is unknown. R39.81, functional urinary incontinence, is a specific type. It applies when the bladder itself functions normally but the patient cannot reach the toilet in time because of a physical or cognitive impairment such as dementia or severe arthritis. R32 and R39.81 each carry an Excludes1 note for the other and are not reported together for the same condition. When functional incontinence is the documented type, R39.81 applies.
No. R32 does not map to any HCC category under CMS-HCC v28, and it did not map to one under CMS-HCC v24. It contributes no RAF.
R32 is not reported alongside N39.3 or N39.41 for the same condition. The R32 Excludes1 note names stress incontinence and other specified urinary incontinence (the N39.3 and N39.4 codes). When the type is documented, the specific code replaces R32.
R32 includes enuresis NOS. Nonorganic enuresis is coded F98.0 in the psychiatric chapter. F98.0 and R32 each carry an Excludes1 note for the other and are not reported together for the same condition.
The note must show urinary incontinence documented without a stated type, such as urinary incontinence NOS or enuresis NOS. R32 also applies when the record lacks the detail needed to distinguish stress, urge, functional or another type. Where a provider query is available, query for the type before assigning R32 by default.
R32 may be appropriate at an initial outpatient encounter when the type has not yet been established through urodynamic or clinical evaluation. On the inpatient side, update the discharge diagnosis to the specific type where evaluation during the admission determined it.
No. R32 is unchanged in the FY2027 update, effective 1 October 2026. The FY2027 addenda make no change to R32 or to its Excludes1 note.
