Which R10 code do I assign?
Category R10 splits on where the pain is documented and on the character of the pain, not on how severe it is. R10.9 is the residual code, correct only when the note gives no site and no established diagnosis that explains the pain.

Why specificity matters beyond the chart: risk adjustment, DRG assignment and denial exposure all follow from which code is selected.

FAQ
Yes. R10.9 is a valid code for HIPAA-covered transactions. It was valid in the FY2026 code set and is unchanged in the FY2027 update, effective 1 October 2026.
Use R10.84 when the provider documents diffuse, generalized or widespread abdominal pain without a specified site. Use R10.9 only when no site information is available at all. A note reading abdominal pain with no qualifier defaults to R10.9.
No. R10.9 does not map to any HCC under CMS-HCC v28. It did not map to one under CMS-HCC v24 either. Only an established underlying diagnosis can carry an HCC mapping.
The note must document three things. Abdominal pain as a symptom or clinical finding. The absence of an established underlying etiology, or a statement that the etiology cannot yet be determined. The absence of any documented anatomic site, since a site would direct the encounter to a more specific code within category R10.
No. Under the ICD-10-CM Chapter 18 guidelines, codes from R00 to R99 are not assigned when an established diagnosis that fully explains the symptom is available. Assign R10.9 only when no definitive underlying diagnosis has been coded that subsumes the abdominal pain.
On the outpatient side, R10.9 is the standard assignment for an undifferentiated abdominal pain presentation where no etiology has been established at the time of service. On the inpatient side, an underlying condition identified or confirmed before discharge becomes the principal diagnosis. Where the workup is negative and the cause remains unidentified, R10.9 or a more specific R10 code stands as the discharge principal diagnosis.
R10.9 itself was not revised. In FY2026, subcategory R10.2 was split into R10.20 through R10.24, per the CMS MS-DRG Definitions Manuals v42.0 (FY2025) and v43.0 (FY2026). Category R10 carries an Excludes2 note for costovertebral (angle) tenderness (R39.85), which is coded to the R39.85 codes rather than absorbed into an abdominal pain code. R10.9 is unchanged in the FY2027 update, effective 1 October 2026.
Yes. The note at category R10 for R39.85 is an Excludes2, which permits both codes when both findings are independently documented and clinically appropriate. Costovertebral angle tenderness is a distinct finding from unspecified abdominal pain.
