M67.A09

M67.A09: Plantar fasciitis, unspecified foot

The ICD-10-CM code for plantar fasciitis is M67.A09 where the record names the diagnosis without naming a side. The code is new in FY2027, effective 1 October 2026, and replaces the M72.2 assignment that applied through 30 September 2026.

Condition

Plantar fasciitis

Family Specificity

Subcategory M67.A0 splits on side alone. M67.A01 is the right foot, M67.A02 the left foot, M67.A09 the foot with no side stated. The subcategory carries no bilateral code.

Effective Date

1 October 2026. M67.A09 is new in FY2027. Plantar fasciitis was reported with M72.2 through 30 September 2026.

Which M67.A0 code do I assign?

Subcategory M67.A0 splits on one axis only, the side of the body. The remaining decision for a coder who has already settled the diagnosis is which foot the record names.

When documentation says…
Code
"Right plantar fasciitis" or "plantar fasciitis of the right heel"
M67.A01
Plantar fasciitis, right foot. The note has to place the diagnosis at the plantar fascia and name the right side. Section I.B.13 of the Official Guidelines permits the side to be taken from the documentation of clinicians other than the patient's provider.
"Left plantar fasciitis" or "left heel plantar fasciitis"
M67.A02
Plantar fasciitis, left foot. The same two requirements apply, with the left side named. Conflicting documentation on the side calls for a provider query.
"Bilateral plantar fasciitis" or "plantar fasciitis of both heels"
M67.A01 and M67.A02
Both unilateral codes, reported together. Subcategory M67.A0 carries no bilateral code. Section I.B.13 directs that separate codes for the left and the right side are assigned where no bilateral code is provided. M67.A09 is the unspecified side code. It is not a bilateral code.
"Plantar fasciitis" with no side stated anywhere in the record
M67.A09
Plantar fasciitis, unspecified foot, which carries plantar fasciitis NOS as its FY2027 inclusion term. Section I.B.13 states that unspecified side codes should rarely be used, such as when the documentation cannot establish the affected side and clarification cannot be obtained. A query for the side is the better step.
"Ledderhose disease", "plantar fibroma" or "nodular thickening within the plantar fascia"
M72.20
Plantar fascial fibromatosis, unspecified foot. FY2027 deleted plantar fasciitis as an inclusion term under M72.2 and added Ledderhose disease in its place. The two conditions are no longer classified together. M72.21 is the right foot and M72.22 the left foot.
"Heel spur" or "calcaneal spur on lateral radiograph"
M77.30
Calcaneal spur, unspecified foot, with M77.31 for the right foot and M77.32 for the left foot. Category M67 carries no Excludes note naming the M77.3 codes. Category M77 carries no Excludes note naming M67.A0. A plantar fasciitis and a calcaneal spur documented in the same record are reported with both codes.
"Heel pain" with no established cause by the end of the encounter
M79.673
Pain in unspecified foot, with M79.671 for the right foot and M79.672 for the left foot. A documented plantar fasciitis displaces the symptom code. Section I.B.5 of the Official Guidelines directs that signs and symptoms associated routinely with a disease process are not assigned as additional codes.

Tabular notes

INCLUDES

Code M67.A09 carries one inclusion term: plantar fasciitis NOS. FY2027 placed it at the unspecified foot code rather than at the M67.A0 subcategory header. A record reading plantar fasciitis with no further qualification and no side therefore reaches M67.A09 directly. Subcategory M67.A0 and subcategory M67.A carry no inclusion term of their own. Category M67 carries no Includes note.

EXCLUDES1

Category M67 carries three Excludes1 notes, which M67.A09 inherits: palmar fascial fibromatosis [Dupuytren] (M72.0), tendinitis NOS (M77.9-) and xanthomatosis localized to tendons (E78.2). Excludes1 means an excluded condition is not reported together with a code from category M67 for the same condition. A documented Dupuytren contracture is assigned M72.0 and not a code from M67. Subcategory M67.A, subcategory M67.A0 and code M67.A09 carry no Excludes1 note of their own. The Excludes1 note that FY2027 revised sits elsewhere in the tabular. Code M72.8, other fibroblastic disorders, carried plantar fasciitis (M72.2) in its Excludes1 list through 30 September 2026. FY2027 revised that entry to plantar fasciitis, the M67.A0 codes. A documented plantar fasciitis is therefore reported with an M67.A0- code rather than M72.8.

EXCLUDES2

Code M67.A09 carries no Excludes2 note. Subcategory M67.A0, subcategory M67.A and category M67 carry none either. Chapter 13 carries a chapter level Excludes2 list, two entries of which bear on the heel: injury, poisoning and certain other consequences of external causes (S00-T88), and symptoms, signs and abnormal clinical and laboratory findings, not elsewhere classified (R00-R94). Excludes2 means the patient may carry both conditions at once. A traumatic rupture of the plantar fascia is an injury and is classified in chapter 19 rather than at M67.A09.

USE ADDITIONAL

Code M67.A09 carries no Use Additional instruction. Subcategory M67.A0, subcategory M67.A and category M67 carry none either. The code carries no Code Also instruction and no Code First instruction. Chapter 13 opens with a chapter level note rather than a formal Use Additional instruction. It reads: use an external cause code following the code for the musculoskeletal condition, if applicable, to identify the cause. Where the record documents an activity behind the fasciitis, an activity code from category Y93 supports that note. Y93.02 is activity, running. Y93.01 is activity, walking, marching and hiking.

FY2027: EFFECTIVE 1 OCTOBER 2026

M67.A09 is one of 190 codes added in FY2027. The same addendum recorded 30 deletions and 4 revisions, taking the code set from 74,719 codes to 74,879. Five of the additions build the new plantar fasciitis family: subcategory M67.A, plantar fasciitis, subcategory M67.A0, plantar fasciitis, foot, and the three billable codes M67.A01 for the right foot, M67.A02 for the left foot and M67.A09 for the unspecified foot. The same release restructured M72.2. Plantar fasciitis was deleted as an inclusion term there. Ledderhose disease was added in its place. M72.2 gained three children: M72.20 for the unspecified foot, M72.21 for the right foot and M72.22 for the left foot. M72.2 was removed from the billable code list in the same update and is now a non-billable header. The tabular entry for M72.2 itself was not deleted. The third change is at M72.8, whose Excludes1 entry was revised from plantar fasciitis (M72.2) to plantar fasciitis, the M67.A0 codes. Encounters dated 1 October 2026 or later report plantar fasciitis with a code from the M67.A0- group. Encounters dated 30 September 2026 or earlier report it with M72.2.

Operational & financial impact

Why specificity matters beyond the chart: risk adjustment, DRG assignment and denial exposure all follow from which code is selected.

CMS-HCC v28 (PY2024+)

M67.A09 has no established CMS-HCC v28 mapping. The code was created for FY2027 and postdates the published CMS-HCC v28 crosswalk. Its mapping should be confirmed against the current CMS-HCC v28 model files rather than assumed from the crosswalk's silence.

The crosswalk bundled with the CMS-HCC v28 model software covers FY2022 and FY2023. No code in category M67 appears in it. No code in category M77 appears in it either. M72.2, the code that carried plantar fasciitis through 30 September 2026, is absent from it as well.

One code in the category plantar fasciitis left behind does map. M72.6, necrotizing fasciitis, maps to HCC 92, bone, joint, muscle and severe soft tissue infections and necrosis, in that same published crosswalk.

Chapter 13 codes in that crosswalk map to ten v28 categories. Among them are HCC 92, bone, joint, muscle and severe soft tissue infections and necrosis, HCC 93, rheumatoid arthritis and other specified inflammatory rheumatic disorders, and HCC 94, systemic lupus erythematosus and other specified systemic connective tissue disorders. A mechanical fasciitis of the plantar fascia matches none of those three descriptions.

Moving from M67.A09 to M67.A01 or M67.A02 is a compliance and medical necessity gain. No RAF figure is attributed to the code here. The code postdates the published crosswalk.

MS-DRG v44 (FY2027)

MS-DRG 557: tendonitis, myositis and bursitis with MCC.

MS-DRG 558: tendonitis, myositis and bursitis without MCC.

An inpatient admission with M67.A09 as the principal diagnosis groups to MS-DRG 557 or 558 in MDC 08 under the FY2027 MS-DRG v44 grouper, per Appendix B of the CMS MS-DRG v44.0 Definitions Manual. Both sit in the medical partition.

The pair splits on MCC alone. There is no CC tier between 557 and 558. Severity weight on such an admission comes from comorbidities documented elsewhere in the record.

Where the admission is driven by a heel infection, a calcaneal fracture or a rupture of the plantar fascia, that diagnosis is sequenced first. M67.A09 becomes a secondary diagnosis. Osteomyelitis of the ankle and foot coded to M86.17-, M86.27- or M86.67- groups to MS-DRG 539, 540 or 541, osteomyelitis. A calcaneal fracture, initial encounter (S92.0-), groups to MS-DRG 562 or 563.

Medicare and commercial payers

M67.A09 is a valid billable code. It requires no further characters.

Section I.B.13 of the Official Guidelines states that codes for the unspecified side should rarely be used. It gives as its example documentation that is insufficient to determine the affected side when clarification cannot be obtained.

Section I.C.13 states that most chapter 13 codes carry site and laterality designations.

Laterality is a new exposure on this diagnosis. Through 30 September 2026 no ICD-10-CM code for plantar fasciitis carried a laterality character. A denial written against M72.2 for a missing side had no basis in the code set. From 1 October 2026 the side is available at M67.A01 and M67.A02. A record that names the foot and a claim that reports M67.A09 now disagree.

Orthotic, injection, imaging and physical therapy policies are written against a named condition. M67.A09 supplies the condition without the side.

Claims spanning the release split on the date of service. An encounter dated 30 September 2026 reports M72.2. An encounter dated 1 October 2026 reports a code from the M67.A0- group. A claim carrying M72.2 for a date of service on or after 1 October 2026 reports a code that is no longer on the billable list.

WHERE SPECIFICITY IS LOST

M67.A09 gives up the side. The provider examined a heel and knows which one. The unspecified side code therefore records a gap in the note rather than a gap in clinical knowledge.

The side costs one word to recover. M67.A01 and M67.A02 need nothing more than right or left in the assessment.

Bilateral involvement is the second loss. Subcategory M67.A0 carries no bilateral code. Both heels take M67.A01 and M67.A02 together. M67.A09 is not a bilateral code.

The heel spur is a separate finding with a separate code. Calcaneal spur is classified to M77.30 for the unspecified foot, M77.31 for the right foot and M77.32 for the left foot. Subcategory M77.3 carries laterality. Reporting M67.A09 alone where imaging documents a calcaneal spur drops a coded finding that does carry a side.

Where the heel pain has no established cause, the foot pain codes are the correct report. M79.673 covers the unspecified foot, M79.671 the right foot and M79.672 the left foot. A documented plantar fasciitis displaces them under section I.B.5 of the Official Guidelines.

M72.6, necrotizing fasciitis, sits in the category plantar fasciitis left behind. It describes a rapidly progressive infection with tissue necrosis. It maps to HCC 92. Nothing in the old category heading fibroblastic disorders separated the two clinically. The descriptor has to be read before the code is selected.

FAQ

Is M67.A09 a billable code?

Yes. M67.A09 is a valid billable code in the FY2027 code set and requires no further characters. Its two parent codes are non-billable headers: M67.A, plantar fasciitis, and M67.A0, plantar fasciitis, foot.

What was the ICD-10 code for plantar fasciitis before 1 October 2026?

Before 1 October 2026, plantar fasciitis was coded to M72.2, plantar fascial fibromatosis, which carried plantar fasciitis as an inclusion term. FY2027 deleted that inclusion term and created the M67.A0- codes in its place. M72.2 was also removed from the billable code list and is now a non-billable header. An encounter dated 30 September 2026 or earlier still reports M72.2.

When should I use M67.A09 instead of M67.A01 or M67.A02?

M67.A09 applies only where the record does not establish the affected foot and the documentation cannot be clarified. Section I.B.13 of the Official Guidelines states that codes for the unspecified side should rarely be used. The side may be taken from the documentation of clinicians other than the patient's provider. For bilateral plantar fasciitis, M67.A01 and M67.A02 are reported together. Subcategory M67.A0 carries no bilateral code. M67.A09 is not a substitute for a bilateral code.

How is M67.A09 different from M72.20?

FY2027 made them different conditions. M67.A09 is plantar fasciitis, a degenerative and inflammatory disorder of the plantar fascia at its calcaneal origin. M72.20 is plantar fascial fibromatosis of the unspecified foot, also called Ledderhose disease, a nodular fibrous proliferation within the fascia. FY2027 removed plantar fasciitis as an inclusion term under M72.2 and added Ledderhose disease. The two are no longer synonyms in the tabular.

Does M67.A09 map to a CMS-HCC v28 category?

No mapping is established. M67.A09 was created for FY2027 and postdates the published CMS-HCC v28 crosswalk, which covers FY2022 and FY2023. Its mapping should be confirmed against the current CMS-HCC v28 model files. No code in category M67 appears in the published crosswalk.

Do I code the heel spur separately?

Yes. A documented heel spur is coded separately. Calcaneal spur is classified to M77.30 for the unspecified foot, M77.31 for the right foot and M77.32 for the left foot. Category M67 carries no Excludes note naming the M77.3 codes. Category M77 carries no Excludes note naming M67.A0. Both codes are reported where both findings are documented.

Should I still report M79.673 for the heel pain?

No. A documented plantar fasciitis displaces the symptom code. Section I.B.5 of the Official Guidelines directs that signs and symptoms associated routinely with a disease process are not assigned as additional codes. The foot pain codes are the correct report only where no diagnosis explaining the pain has been established by the end of the encounter.

How does M67.A09 group on an inpatient claim?

An inpatient admission with M67.A09 as the principal diagnosis groups to MS-DRG 557 with MCC or MS-DRG 558 without MCC, tendonitis, myositis and bursitis. Both sit in MDC 08 under the FY2027 MS-DRG v44 grouper, per Appendix B of the CMS MS-DRG v44.0 Definitions Manual. Where the admission is driven by a heel infection, a calcaneal fracture or a rupture of the plantar fascia, that diagnosis is sequenced first and M67.A09 becomes a secondary diagnosis. On the outpatient side only a confirmed diagnosis is coded.
