Which M41 code do I assign?
Category M41 splits first on the cause of the curve, then on the region of the spine it occupies. M41.9 is the residual code for the record that settles neither axis.

Why specificity matters beyond the chart: risk adjustment, DRG assignment and denial exposure all follow from which code is selected.

FAQ
Yes. M41.9 is a valid billable code and requires no further characters. It is the most specific code available when the record documents neither the cause of the curve nor the spinal region. The alphabetic index main term Scoliosis (acquired) (postural) leads to M41.9. The rule is the same in the inpatient and the outpatient setting. An outpatient encounter codes only the confirmed diagnosis. An inpatient discharge diagnosis documented as probable or suspected may be coded as though confirmed under UHDDS rules.
Category M41 carries seven cause subcategories before the unspecified code: M41.0- for infantile idiopathic scoliosis, M41.1- for juvenile and adolescent idiopathic scoliosis, M41.2- for other idiopathic scoliosis, M41.3- for thoracogenic scoliosis, M41.4- for neuromuscular scoliosis, M41.5- for other secondary scoliosis and M41.8- for other specified forms. The idiopathic group splits again at the fifth character, M41.11- for juvenile and M41.12- for adolescent. Adolescent idiopathic scoliosis therefore has its own codes and never belongs at M41.9. ICD-10-CM states no age boundary in the tabular. The Scoliosis Research Society places infantile idiopathic scoliosis at ages 0 to 3, juvenile at ages 4 to 10 and adolescent at ages 11 to 18. StatPearls puts the prevalence of adolescent idiopathic scoliosis at 1 percent to 3 percent.
The spinal region is carried by the last character of the code. The region values run: 0 for site unspecified, 1 for the occipito-atlanto-axial region, 2 for cervical, 3 for cervicothoracic, 4 for thoracic, 5 for thoracolumbar, 6 for lumbar, 7 for lumbosacral and 8 for the sacral and sacrococcygeal region. That character is the fifth in the M41.0-, M41.2-, M41.3-, M41.4-, M41.5- and M41.8- codes. It is the sixth in the M41.11- and M41.12- codes, because the fifth character there already separates juvenile from adolescent. Two exceptions matter. In M41.11- and M41.12- the site unspecified value is 9 rather than 0. No subcategory carries every region value: only M41.4- uses 1, only M41.0- uses 8, and M41.3- offers 0, 4 and 5 alone.
Use M41.20 whenever the record states idiopathic scoliosis and gives no age of onset and no region. The alphabetic index routes idiopathic scoliosis to M41.20. M41.20 is the nearest wrong choice to check before assigning M41.9, because the single word idiopathic is enough to move the encounter out of M41.9. Where the note gives an age of onset, the M41.0-, M41.11- or M41.12- codes apply instead.
No. Category M41 carries an Excludes1 note for congenital scoliosis NOS (Q67.5), for postural congenital scoliosis (Q67.5) and for congenital scoliosis due to bony malformation (Q76.3). Excludes1 means the two are not reported together for the same condition. A curve documented as congenital is coded from chapter 17. Code Q76.3 covers hemivertebra fusion or failure of segmentation with scoliosis. Code Q67.5 covers congenital spinal deformity with no documented bony malformation. Code Q67.5 carries the reciprocal Excludes1 note for infantile idiopathic scoliosis (M41.0), which keeps an idiopathic infant curve inside M41.
Kyphoscoliosis codes to M41. Category M41 carries an Includes note for kyphoscoliosis. Category M40, kyphosis and lordosis, carries the reciprocal Excludes1 note for kyphoscoliosis, the M41 codes. Kyphosis alone is coded from the M40.0- through M40.3- codes. Lordosis alone is coded from the M40.4- and M40.5- codes. Kyphoscoliosis with no documented cause and no documented region lands on M41.9. One exclusion sits on top of this. Where the record documents heart disease caused by the kyphoscoliosis, the encounter takes I27.1, which M41 excludes under an Excludes1 note.
No. M41.9 carries no CMS-HCC v28 mapping. The v28 model has no category for scoliosis or for spinal deformity. Coding a more specific scoliosis code does not change a Medicare Advantage risk score. The inpatient grouper is a separate question. M41.9 sits on the spinal curvature diagnosis list for MS-DRG 456, 457 and 458, which apply when a noncervical spinal fusion is performed. A medical admission with M41.9 as the principal diagnosis and no spinal procedure groups to MS-DRG 551 or 552 in MDC 08. As a secondary diagnosis M41.9 is reported when the scoliosis is documented and addressed during the admission.
M41.9 is unchanged in the FY2027 update, effective 1 October 2026. The FY2026 update, effective 1 October 2025, also left the descriptor at M41.9 and the Includes and Excludes notes at M41 unchanged. No code in categories M40 through M43 was added, deleted or revised in either update.
