Which L20 code do I assign?
The L20 family splits on the named clinical subtype the record states. It does not split on severity, body site or laterality. A severe or widespread presentation stays on the same code as a mild one unless a subtype is named.

Why specificity matters beyond the chart: risk adjustment, DRG assignment and denial exposure all follow from which code is selected.

FAQ
Yes. L20.9 is a valid code for submission under the FY2027 code set, effective 1 October 2026. Category L20 and subcategory L20.8 are headers rather than complete codes. Neither of those is billable.
L20.9 cannot be assigned on that wording alone. The Alphabetic Index main term Eczema, carrying the nonessential modifiers (acute) (chronic) (erythematous) (fissum) (rubrum) (squamous), leads to L30.9, dermatitis unspecified. The tabular prints Eczema NOS as an inclusion term under L30.9. L20.9 requires the record to state that the dermatitis is atopic. The L20–L30 block note that dermatitis and eczema are synonymous governs the two words, not the specificity of the diagnosis.
The nearest code is L30.9, dermatitis unspecified. It is where the Alphabetic Index sends unqualified eczema and unqualified dermatitis. L28.0, lichen simplex chronicus, is the other near neighbor. Its inclusion terms are circumscribed neurodermatitis and lichen NOS. Subcategory L20.8 carries an Excludes2 note pointing to it.
No. No code in category L20 maps to a payment HCC in the CMS-HCC V28 model. V28 is the 2024 CMS-HCC model. CMS calculates 100 percent of Medicare Advantage risk scores from it for payment year 2026. The skin categories in V28 cover pressure and chronic ulcers, severe skin burn, and specified autoimmune skin disorders only.
The note has to name a subtype. Flexural involvement supports L20.82. Infantile onset supports L20.83. An intrinsic phenotype supports L20.84. Atopic neurodermatitis supports L20.81. A subtype that is named but is covered by none of those supports L20.89.
Sequence on the reason for the encounter. Where the visit is driven by the superinfection, the infection code leads. L20.9 follows as an additional diagnosis. L01.1, impetiginization of other dermatoses, is the code for a secondarily impetiginized dermatitis. L08.9 covers an unspecified local infection of skin and subcutaneous tissue. Add a code from B95 or B96 when the organism is identified. Add B00.0, eczema herpeticum, where herpes simplex superinfection is documented.
The code itself is the same in both settings. Outpatient coding requires a confirmed diagnosis. Probable or suspected atopic dermatitis is not coded there. Inpatient coding may report a diagnosis documented as uncertain at discharge as though it were confirmed, under UHDDS rules. Where L20.9 is the principal diagnosis on an inpatient stay it groups to MS-DRG 606 or 607.
L20.9 is unchanged in the FY2027 update, effective 1 October 2026. The update leaves category L20 and its notes unchanged. Category L20 also gained no codes and lost none for FY2026, which took effect 1 October 2025. No Excludes note in the L20 hierarchy changed that year.
