Condition

Colon cancer

Family Specificity

Ten billable child codes split by anatomic subsite of the colon, with C18.9 as the residual code for a confirmed colon primary whose segment is not documented.

Effective Date

Valid in the FY2026 code set and unchanged in the FY2027 update effective 1 October 2026.

Which C18 code do I assign?

The C18 family splits on the anatomic subsite of the colon named in the operative, endoscopic or pathology report, not on stage, histology or treatment intent. Reserve C18.9 for a confirmed primary colon malignancy whose segment is genuinely absent from the record.

When documentation says…
Code
Adenocarcinoma of the cecum
C18.0
A primary malignancy of the cecum. The tabular carries malignant neoplasm of the ileocecal valve as an inclusion term at C18.0. A lesion described at the ileocecal valve therefore codes here rather than to a small intestine code.
Adenocarcinoma of the ascending colon
C18.2
A primary malignancy of the ascending colon. The two adjacent segments are separate codes. A lesion at the hepatic flexure takes C18.3. A lesion in the transverse colon takes C18.4.
Tumor in the descending colon
C18.6
A primary malignancy of the descending colon. A lesion at the splenic flexure takes C18.5 instead. The note must name the segment, because descending and sigmoid are separate codes.
Tumor in the sigmoid colon
C18.7
A primary malignancy of the sigmoid colon. C18.7 carries an Excludes1 note against malignant neoplasm of rectosigmoid junction. A lesion documented at the junction is coded to C19 rather than C18.7.
Mass at the rectosigmoid junction
C19
Malignant neoplasm of rectosigmoid junction, which sits outside category C18 entirely. The tabular lists malignant neoplasm of colon with rectum and malignant neoplasm of rectosigmoid as inclusion terms at C19. A lesion in the rectum itself is C20.
Tumor spanning the splenic flexure and descending colon
C18.8
Malignant neoplasm of overlapping sites of colon. The Chapter 2 note on primary malignant neoplasms overlapping site boundaries directs a primary tumor crossing two or more contiguous sites to the overlapping lesion code, which for the colon is C18.8. Two separate, non-contiguous colon tumors are coded to each site instead.
Colon cancer with no segment stated anywhere in the record
C18.9
Malignant neoplasm of colon, unspecified, carrying the inclusion term malignant neoplasm of large intestine NOS. Query the surgeon or the pathologist before selecting this code. C18.9 asserts that the segment is unknown. A resection or biopsy report that names a segment contradicts that assertion.

Tabular notes

EXCLUDES1

Category C18 carries one Excludes1 note: malignant carcinoid tumors of the colon (the C7A.02 codes). An Excludes1 note means the two are not reported together for the same condition. A carcinoid or malignant neuroendocrine histology on the pathology report moves the case out of C18 and into the C7A.02- codes. Subcategory C18.7 carries its own Excludes1 note against malignant neoplasm of rectosigmoid junction (C19). Block C15-C26 carries a further Excludes1 note for Kaposi's sarcoma of gastrointestinal sites (C46.4).

EXCLUDES2

Block C15-C26 carries an Excludes2 note for gastrointestinal stromal tumors (the C49.A codes). An Excludes2 note means the patient may have both conditions. Both may be reported when both are documented. A gastrointestinal stromal tumor of the large intestine is classified to C49.A4. It is not reported with a C18 code for the same lesion.

FY2027: EFFECTIVE 1 OCTOBER 2026

Category C18 carries no Use additional instruction and no Code also instruction in the FY2027 tabular. The Excludes notes reaching C18.9 are the Excludes1 note at the category and the Excludes1 and Excludes2 notes at block C15-C26. C18.9 carries the inclusion term malignant neoplasm of large intestine NOS. The FY2027 codes and tabular addenda make no addition, deletion or revision in category C18. FY2027 adds QA1.71, Lynch syndrome. Its category QA1 carries a Code also note for associated malignant neoplasms (C00.0-C96.9).

Operational & financial impact

Why specificity matters beyond the chart: risk adjustment, DRG assignment and denial exposure all follow from which code is selected.

CMS-HCC v28 (PY2024+)

C18.9 maps to HCC 22: Bladder, Colorectal, and Other Cancers.

CMS-HCC v28 is fully phased in at 100 percent for payment year 2026.

Two metastatic categories sit above HCC 22 in the v28 cancer hierarchy. HCC 17 covers cancer metastatic to lung, liver, brain and other organs. HCC 18 covers cancer metastatic to bone together with other and unspecified metastatic cancer.

For Medicare Advantage risk adjustment, an active colon malignancy must be documented and assessed at least once each calendar year. The code must appear on a dated encounter note rather than only on a problem list.

MS-DRG v44 (FY2027)

MS-DRG 374: Digestive Malignancy with MCC.

MS-DRG 375: Digestive Malignancy with CC.

MS-DRG 376: Digestive Malignancy without CC/MCC.

A medical admission with C18.9 as the principal diagnosis groups to MS-DRG 374, 375 or 376 under the FY2027 MS-DRG v44 grouper.

With C18.9 as the principal diagnosis, an admission with a major bowel procedure such as a colectomy groups instead to the surgical set in MDC 06 under the FY2027 MS-DRG v44 grouper. MS-DRG 329: Major Small and Large Bowel Procedures with MCC. MS-DRG 330: Major Small and Large Bowel Procedures with CC. MS-DRG 331: Major Small and Large Bowel Procedures without CC/MCC.

SEQUENCING UNDER FY2027 GUIDELINES CHAPTER 2

Section I.C.2.a: when the malignancy is chiefly responsible for the admission or encounter and treatment is directed at the primary site, the primary malignancy is the principal or first-listed diagnosis. The one stated exception is an encounter chiefly for administration of chemotherapy, immunotherapy or external beam radiation therapy. In that case the Z51.- code is first listed and the malignancy is reported as a secondary diagnosis.

Section I.C.2.b covers a primary neoplasm with metastasis where treatment is directed toward the secondary site only. The secondary neoplasm is designated as the principal diagnosis even though the primary malignancy is still present.

Section I.C.2.l: when the encounter is for treatment of the primary malignancy, the primary site is sequenced first, followed by any metastatic sites.

Section I.C.2.m sets three conditions for moving a former colon primary out of C18 and into category Z85. The malignancy has been excised or eradicated from its site. No further treatment is directed to that site. There is no evidence of any existing primary malignancy at that site.

Medicare and commercial payers

There is no National Coverage Determination that governs diagnosis code selection for colon cancer.

C18.9 supports medical necessity for oncology evaluation and management, chemotherapy administration and surveillance imaging when the malignancy is documented as active.

A screening encounter is reported differently. Z12.11 covers an encounter for screening for malignant neoplasm of colon in a patient with no known malignancy, not an encounter for a diagnosed colon cancer.

These risk adjustment rules apply to Medicare fee for service and to Medicare Advantage plans using CMS-HCC v28 for payment year 2026.

WHERE SPECIFICITY IS LOST

C18.9 states that the colon segment is unknown. When the operative report or the pathology report names a segment, the claim and the record disagree. That mismatch is what a coding audit finds.

Omitting the secondary neoplasm codes in metastatic disease leaves the encounter in HCC 22 when the documented metastases would support HCC 17 or HCC 18 under CMS-HCC v28.

Reporting C18.9 for a resolved colon cancer overstates active disease. Section I.C.2.m directs Z85.038 for a former colon primary with no further treatment and no residual disease.

Assigning C18.7 to a rectosigmoid junction tumor contradicts the Excludes1 note at C18.7. The correct code is C19.

SEVERITY DESIGNATION (MS-DRG V44.0)

C18.9 is a CC. It appears in Appendix G, diagnoses defined as complications or comorbidities, in the ICD-10 MS-DRG Definitions Manual Version 44.0. As a secondary diagnosis it can move a case to a with-CC tier where the DRG pair offers one.

FAQ

Is C18.9 a billable ICD-10-CM code?

Yes. C18.9 is a valid billable code in the FY2027 code set, effective 1 October 2026. It is a leaf code with no further subdivision. Billable is not the same as defensible. When the record names the colon segment, a subsite code from C18.0 through C18.7 is the correct assignment.

When should I use C18.9 instead of a named subsite code?

Use C18.9 only when a primary colon malignancy is confirmed and no segment appears anywhere in the operative report, the colonoscopy report or the pathology report. The inclusion term at C18.9 is malignant neoplasm of large intestine NOS. Query the surgeon or the pathologist before defaulting to C18.9. A resection specimen identifies the segment.

What is the difference between C18.7 and C19?

C18.7 is malignant neoplasm of sigmoid colon. C19 is malignant neoplasm of rectosigmoid junction. Subcategory C18.7 carries an Excludes1 note against C19. The two are not reported together for the same condition. The tabular lists malignant neoplasm of colon with rectum and malignant neoplasm of rectosigmoid as inclusion terms at C19. A lesion documented at the junction is C19, not C18.7.

How do I code metastatic colon cancer?

Assign the colon primary, which is C18.9 when no segment is documented. Add a secondary neoplasm code for every documented metastatic site, such as C78.7 for liver, C78.0- for lung, C79.31 for brain or C79.51 for bone. Under FY2027 guideline Section I.C.2.l the primary site is sequenced first, followed by the metastatic sites, when treatment is directed at the primary. Under Section I.C.2.b the secondary neoplasm is designated as the principal diagnosis when the admission is for treatment of the secondary site only, even though the primary malignancy is still present. A separate unrelated primary, such as prostate cancer at C61, keeps its own primary site code and is not reported as a metastasis of the colon tumor.

Does C18.9 map to an HCC?

Yes. C18.9 maps to HCC 22, Bladder, Colorectal, and Other Cancers, under the CMS-HCC v28 model.

When is a colon cancer Z85.038 rather than C18.9?

FY2027 guideline Section I.C.2.m sets three conditions that must all hold. The primary malignancy has been excised or eradicated from its site. No further treatment is directed to that site. There is no evidence of any existing primary malignancy at that site. Where all three hold, assign Z85.038, personal history of other malignant neoplasm of large intestine, which the tabular annotates as conditions classifiable to C18. A patient still receiving adjuvant chemotherapy after resection remains an active C18 case.

Do the inpatient and outpatient rules differ for C18.9?

Code selection is identical. Sequencing and diagnostic certainty differ. On the inpatient side the principal diagnosis is selected under UHDDS rules and FY2027 guideline Section I.C.2. In the outpatient setting only a confirmed malignancy is coded. A suspected, probable or rule out colon cancer is not coded as confirmed in the outpatient setting.

What changed for C18.9 in FY2027?

C18.9 is unchanged in the FY2027 update. The FY2027 update, effective 1 October 2026, makes no addition, deletion or revision in category C18. The Excludes1 note against malignant carcinoid tumors of the colon, C7A.02-, carries forward unchanged.
